Release of Liability & Media Consent

In consideration of being permitted to board and participate in activities aboard the vessel D/V Chelsea Rae (the "Vessel"), including but not limited to boating, marine research, shark tagging, and related on-water activities (the "Activity"), I acknowledge and agree to the following on behalf of myself, my heirs, and my personal representatives:

Assumption of Risk. I understand that boating and marine activities involve inherent and other risks, including rough water, weather, equipment, wildlife, slips and falls, injury, illness, drowning, property damage, and even death. I knowingly and voluntarily assume all such risks, both known and unknown, and accept full responsibility for my participation.

Release & Waiver. I hereby release, waive, discharge, and hold harmless All Points Boats, LLC, the owner(s), operator(s), captain, and crew of the Vessel, together with any partnering organizations and their staff, volunteers, agents, officers, and directors (collectively, the "Released Parties"), from any and all liability, claims, demands, or causes of action for any injury, loss, or damage to my person or property arising out of or related to the Activity, whether caused by the negligence of the Released Parties or otherwise, to the fullest extent permitted by law.

Fitness & Instructions. I confirm I am physically able to participate and will follow all safety instructions given by the captain and crew.

I grant the Released Parties and their designees the irrevocable, perpetual, royalty-free right to photograph and record me, and to use my name, likeness, image, voice, and any footage or photographs in which I appear (the "Media"), in any and all media now known or later developed, for any purpose including marketing, promotion, social media, and publicity.

I waive any right to inspect or approve the Media and any right to compensation. I agree that I may not demand that any such Media be withheld, removed, or restricted from release. All Media remains the sole property of the Released Parties.
